HCHD Foundation presents check to Harris Health
The Trustees of the Harris County Hospital District Foundation (HCHD Foundation) in its May 8, 2019 Annual Board Meeting, proudly presented a check for $150,000.00 to Harris Health System in support of the Harris Health’s Ambulatory Care Health Hub Demonstration Project. The goal of the Health Hub Demonstration Project is to coordinate more effectively with community partners to comprehensively address the patients’ health-related social needs. The HCHD Foundation looks forward to continuing to care for the Harris County community through partnership with Harris Health System.
