Multiple studies show that patients with chronic heart conditions benefit greatly from comprehensive support and education, which allows for active participation in their care, fewer visits to specialists, and promotes self-efficacy. Hypertension, the leading cause of cardiovascular and coronary mortality issues, requires ongoing management and education. The Heart Health initiative will address these needs by providing resources and support to encourage regular health monitoring and proactive management of heart health.
